I've had a dual monitor setup at work for about 8 years....maybe about 6 at home since work spoiled me.
The one thing that is a must for me is a multi-monitor program. I think mulit-mon or ultra-mon is the one i have always used. There are probably better ones out there. There are additional buttons on each window that give you more options than minimize/maximize/close (they add buttons like switch to other side).
These programs usually also allow for different backgrounds on different monitors. Without this it depends on your operating system. On windows XP i think I would set up a co-worker with a web something option (I forget) but it loads another picture on top of the desktop picture (on one side, therefore giving you two pictures)
